  • Provide skilled physical therapy interventions to improve mobility, strength, function, balance, and pain management
  • Establish measurable goals and modify treatment plans based on patient progress
  • Complete evaluations, progress reports, treatment notes, re-evaluations, and discharge documentation accurately and on time
  • Educate patients and families regarding diagnoses, treatment plans, injury prevention, and home exercise programs
  • Communicate effectively with physicians, case managers, and healthcare professionals
  • Maintain compliant documentation according to state regulations and payer guidelines
  • Promote patient safety and maintain a clean treatment environment
  • Participate in continuing education and quality improvement initiatives
  • Foster positive patient relationships and contribute to a patient-centered culture
